We collect the above data to manage your physiotherapy treatment. It enables us to identify you, contact you and to make appropriate onward referrals, if required and with prior agreement from you, to general practitioners, consultants or other healthcare professionals.
Security of information.
All information is stored securely, and only authorised Axon Physio Ltd representatives can access this information. We will not sell or rent or give your information to third parties. Records are required to be kept for 8 years after the date of the last treatment for adults and 8 years after their 18th birthday for children. After which they are destroyed.
How can you access and update your information?
You can request a copy of the data held about you at any time (GDPR call this “Right to Access”). To do so, please contact us directly on 01277 625652 or info@axonphysio.com. This can be arranged free of charge and once your identity has been verified, the requested data will be provided within 28 days.

Telehealth or Virtual Consultations
We use Cliniko’s integrated telehealth software to provide our virtual or online consultations. This is to ensure sessions are fully encrypted and secure.
